When in an existing action plan you can either revise the current plan or replace the current plan to a completely new plan.

Revising a plan only edits the existing plan. You can add/replace tasks and adjust the numbers whereas Replace will take you to the future numbers screen where you will override the existing action plan and start from scratch.
